This official witnesses the signing of the document and certifies that the signer has affirmed or sworn to the accuracy and truthfulness of its contents under oath. The jurat usually includes details such as the date, the location, and the official’s signature, and it often begins with phrasing like “Subscribed and sworn to before me…” If no other format is prescribed, a Jurat form may be used when an individual is signing and swearing (or affirming) that certain written statements are true.
